Aims of the study
In a community midwifery service, it became apparent that because of late separation of the umbilical cord, midwives were spending a significant amount of their time visiting women after the tenth postnatal day. A retrospective study was begun of all babies seen between March and September 1984 (in total 1503).


Of these, 256 babies were excluded as the day of cord separation was not stated in the community records. It was found that community midwives made 331 visits after the tenth day to examine or treat the cord or stump. In 11% of cases, the cord had separated by the sixth day, while a further 20% did not separate until after the tenth day.


We found that one midwife always recorded the cord off before the sixth postnatal day. Further investigation revealed that her treatment was no different from the other midwives, although we did notice that a majority of her babies were delivered at a particular District General Hospital.


We looked into the cord care of each local hospital, and found it to be similar in all but one respect: babies delivered at our hospital had their cords sprayed with Polybactrin at delivery. It was decided to discontinue this practice for six months, then to make a follow-up survey of the effects on cord separation time and any potential effects on umbilical cord infection.


Community records for the months July to September 1985 were then studied in a second retrospective study. We found that a total of 1097 babies had been attended by midwives. Of these, were excluded from the study as the date of cord separation was not stated. The midwives made visits after the tenth day to treat the umbilical cord, which was a 50% reduction on the previous study's visits. During the period of the second study, 42% of the cords had separated by the sixth day, while only 6% had not separated by the tenth day.


There were no cases of umbilical cord infection. We believe this study supports previous work, and appears to confirm that applying Polybactrin spray to the cord at delivery causes a delay in separation, with no noticeable benefits in reduction of umbilical infection. Polybactrin spray was not reintroduced into the hospital delivery practice.
